I. Proposal & Application

Complete the grant proposal and application form. You will need to upload completed budget request (directions in step II) and signature sheets (directions in step III)

II. Budget Request Form

Complete the Budget Proposal form. You will need to save a copy to your Google drive so you can edit it. When you are finished editing, download and save it as a PDF for uploading with your application.

The itemized budget should list funds requested from ICTM, district financial support, and other sources of funding.

Upload the final version with your application.

III. Principal’s Declaration of Support

Complete the principal's signature form indicating support for your application.

IV. Final Report Requirements

Teaching & Learning Grant recipients will write a short newsletter article (300 - 500 words), with photos and captions, describing how the grant benefits students, families, teachers and or the mathematics department at your school or make 2-3 minutes of short videos (four to six 30 second videos) which show purchases being used or the activity the money was used to fund. (Please blur student faces). Please note if AI was used to help edit or polish your writing. These will be used on the ICTM website and/or social media platforms.

ICTM Conference Grant recipients are to write a short newsletter article (300 - 500 words), with photos and captions, describing their experience and any valuable takeaways from the conference or make 2-3 minutes of short video “commercials” (four to six 30 second videos) sharing their experiences at the conference and using key takeaways. (Please blur student faces). Please note if AI was used to help edit or polish your writing. Recipients are also requested to share from the conference using the hashtag #IowaMathTeach. These will be used on the ICTM website and/or social media platforms.

NCTM Conference Grant recipients are to write a short newsletter article (300 - 500 words), with photos and captions, describing their experience and any valuable takeaways from the conference or make 2-3 minutes of short video “commercials” (four to six 30 second videos) sharing their experiences at the conference and using key takeaways. V. Reimbursement

Reimbursement of expenses will be paid in a timely fashion upon the grant awardee

Reimbursement will be sent to the primary grant applicant or to a designated recipient. Variation from the items in the budget request must be approved prior to expenditure and subsequent reimbursement.
